To Add yourself, choose "Add" on the Status of Form drop down menu and follow the steps.

To change your Communication Preferences, choose "Switch Communication Preferences" and follow the steps. This selection is if you would like to switch from Phone call to email, or from email to text message, etc.

To change your contact information, choose "Change Information" and follow the steps. This selection is for if you have a new phone number or email address.

To opt-out of receiving any communication from Madison United Methodist Church, select Opt-Out and enter your information.
